Finding an archived patient's GFE

To locate patients seen prior to November 1, 2023 please see below.


(Step 1) Please click on the "archived patients" button that can be found on the right hand side of your client portal. A new tab will open for you to search for your patient's GFE.

(Step 2) From there you will be able to search for the patient's GFE you are looking for by entering in any combination of their patient identifiers and then clicking the search button. If you would like to see all archived records you can simply click the search button with all the fields empty and that will bring up all of your archived records.
