For any minors (ages 13-17 years old) requesting aesthetic treatments, the minor’s parent and/or legal guardian must be physically present to provide consent for treatment during the Good Faith Evaluation (GFE) process. *Patients under the age of 13 may NOT be evaluated for any aesthetic treatments at any time. In the event a patient <13 years old enters a GFE visit, the Aesthetic Telehealth Provider should speak with the on-site staff to inform them we are unable to complete a GFE due to the patient's age and end the visit without submitting a deferral.*
Prior to beginning the GFE, the Aesthetic Telehealth Provider must obtain verbal consent from parent and/or legal guardian, document parent and/or legal guardian’s full name, as well as the parent and/or legal guardian’s contact information on GFE form.
Aesthetic Telehealth Providers may provide an approval (if no other contraindications present) to minor patients (13-17 years old) for the following list of elective aesthetic treatments. Any treatment NOT on this list cannot be approved and will ALWAYS be a deferral.
