  1. Welcome to your completed Spakinect Good Faith Evaluation (GFE) document! At the top of the document, you will find the date of the Good Faith Evaluation (GFE), patient identifiers (name and date of birth), and the aesthetic telehealth provider’s credential information.
  1. Health History

  1. In this section, you will find the patient’s pertinent medical information including demographics, allergies, medications, vitamins and supplements, vaccines, relevant medical history, and any previous treatment history.
  2. Patient’s surgical history and permanent implants can be found displayed in chart format. 
  1. Recommended Treatment Summary

  1. Let’s talk treatments! In this section you will find an overview of the treatments that have been approved, conditionally approved, and deferred.
    1. Conditional Approvals: If a treatment(s) is conditionally approved, the condition will be detailed directly below the list of conditional approved treatments. Please ensure that the patient has met the criteria of the condition prior to initiating the treatment plan.
    2. Deferred Treatments: If a treatment(s) is deferred, the deferral will be listed along with instructions for where to find additional information about the deferral.
  1. Evaluation Findings

  1. Let us investigate for you! In the evaluation findings section, you will see any positive exam findings and/or pertinent information about the patient’s body system(s)/area(s) from our provider’s thorough aesthetic assessment as well as the relevant treatment precautions.
  1. Telehealth Provider Note

  1. In certain instances it may be necessary for our expert team of aesthetic telehealth providers to add an additional clinical note about the patient for your clinic staff to review prior to treatment.  
  1. Treatment Plans

  1. Each individually approved or conditionally approved treatment will generate a treatment plan that contains indications for use, device or product information pertinent to the treatment, or important clinical recommendations.
  2. Each treatment plan should be carefully reviewed prior to initiation of treatment as well as subsequent treatments.
  1. Clinical Considerations

  1.  Our Clinical Considerations contain important information regarding our treatment approvals as well as recommendations that we would like our clients to be aware of.
  1. Evaluation Assistance

  1. This section contains documentation of any assistance that was used during the evaluation. Specifically, clients can review interpreter, legal guardian, and Medical Power of Attorney information here.  
  1. Amendments

  1. This section is designed to be utilized when your on-site Medical Director makes a documentation note about the patient and/or treatment. An example of when this area may be used is when a clinic’s medical director overrides a treatment deferral and documents a treatment plan for the patient.
